The registration of birth is done in the district in which the birth occurred and by the following persons: the father/mother and or both parents, the next of kin of either parents, occupier of the premises which the child was born, an adult present at birth or a care taker of the child.

PROCEDURES AND REQUIREMENTS

• Parent of the child must go to the nearest Birth and Death Office in the District where the birth(s) occurred
• Present evidence of birth(s) (i.e. maternal health record book)
• Provide bio-data to the birth event
• Name of child Date of birth (would have already been indicated in the maternal health record book)
• Place of birth (would have already been indicated in the maternal health record book)
• Father’s name, age, occupation, religion, nationality and educational background
• Mother’s name, age, occupation, religion, nationality and educational background
